The purpose of this participant survey is to ensure that Impactful Solutions is providing services in a manner that supports the individuals needs and goals from the participants perspective. Impactful Solutions understands that in some cases a participant is unable to complete this survey unassisted and encourages senior support staff to work with them to complete.
